For some who have experienced a stay in a rehabilitation center or hospital, returning straight home may delay or interfere with a full recovery. To bridge this gap, East Ridge at Cutler Bay, a life plan community for active retirement, has introduced a new initiative, “Bridge to Home,” a post-rehab program that provides the necessary individualized care, hospitality and safety to help expedite someone’s recovery before returning home.

“We have created a structured environment within assisted living at Three Palms that provides our guests with all the comforts of home and supportive care they need. They have time to fully recuperate while they gradually return to their previous lifestyle,” said Marcos Gonzalez, Director of Sales. “Research shows that this model may prevent people from regressing at home and possibly having to return to the hospital. For people without caregivers, this program may be a perfect fit. While living at assisted living, they can enjoy all of the benefits offered residents including access to amenities, services and activities coordinated by a full-time activity director.”

The “Bridge to Home” program at East Ridge includes a comfortable, fully-furnished apartment and three chef-prepared meals served daily. There is 24/7 nursing support including medication management, physician access, dietary consulting, security and an emergency call system and convenient access to physical and occupational therapy within the building.
